Providing a Safety Net for Frail & Vulnerable Seniors Within the Lake Chelan Valley

Our Mission

The Chelan Valley Community Nurse program provides services free of charge to frail and elderly adults in the Chelan Valley who wish to stay in their own homes.

How might a Community Nurse help?

  • Making visits to vulnerable adults living at home.
  • Helping older adults stay safe in their homes
  • Assisting with understanding medications and treatments, especially after hospital discharge.
  • Educating about new conditions and managing chronic conditions.
  • Referring clients to physicians, community resources, and other support services.
  • Offering community blood pressure.

Who could benefit from a visit?

  • A neighbor or loved one who is at risk of falling or might otherwise need help with daily living tasks and living at home.
  • Someone who has recently been discharged from the hospital or has a new diagnosis and could use help managing medications or their condition.
  • Anyone who needs help getting connected to community services because they are hungry, cold, lonely, or in an unsafe living situation.
  • When you have questions or concerns about your or someone else’s health. You can call and talk to a nurse.
